About the Health and Wellbeing Partnership
On 1 July 2022, 42 Integrated Care Systems (ICSs) were established across England. Each ICS consists of an Integrated Care Board and an Integrated Care Partnership. The local Integrated Care Partnership is known as the Leicester, Leicestershire and Rutland Health and Wellbeing Partnership (LLR HWP). It is a statutory committee bringing together an alliance of partners who are concerned with improving the care, health and wellbeing of the local population. It is also responsible for producing an integrated care strategy on how to meet the health and wellbeing needs of the population in the ICS area.
The animation below is aimed primarily at our staff who make up our ICS but is also a great source for anyone who wants to understand the collaborative workings of our partnership.
Our statutory partners
The statutory partners are:
- NHS Leicester, Leicestershire and Rutland Integrated Care Board (from 1 July 2022)
- University Hospitals of Leicester NHS Trust
- Leicestershire Partnership NHS Trust
- East Midlands Ambulance Service
- Leicester City Council
- Leicestershire County Council
- Rutland County Council
GPs, district councils, other health and care providers, Healthwatch and the voluntary and community sector are also important partners.
The LLR Integrated Care Board (ICB) is the statutory NHS organisation responsible for developing a plan for meeting the health needs of the population, managing the NHS budget and arranging for the provision of health services in the ICS area. ICBs have replaced Clinical Commissioning Groups.
Other features of the ICS are:
- Place based partnerships: will lead the detailed design and delivery of integrated services across their localities and neighbourhoods. The partnerships will involve the NHS, local councils, community and voluntary organisations, local residents, services users, their carers, and representatives and other community partners with a role in supporting the health and wellbeing of the population
- Provider collaboratives: will bring NHS and care providers together to improve quality, efficiency and health outcomes and address inequalities in access and experience to services.
- Local authorities: are responsible for social care and public health functions as well as other vital services for local people and businesses. These services include housing, education, early years support, transport: all these services can have an impact on peoples’ health.
Integrated Care
Integrated care is about joining up the care provided by different organisations and services. It’s about giving people the support they need, joined up across local councils, the NHS, and other partners.
Integrated care puts the patient or service user at the centre by removing traditional divisions between hospitals and family doctors, between physical and mental health, and between NHS and council services. In the past, these divisions have meant that too many people experienced disjointed care.
In Leicester, Leicestershire and Rutland there is an excellent track record of joined-up services and partnership working, delivering care built around the needs of local people.
Purpose of ICSs
The purpose of ICSs is to bring partner organisations together to:
- improve outcomes in population health and healthcare
- tackle inequalities in outcomes, experience and access to health and care
- Use the resources available for health and care services to get the most from them
- help the NHS support the broader social and economic development in an area
Working jointly, will help health and care organisations tackle complex challenges, including:
- improving the health of children and young people
- supporting people to stay well and independent
- acting sooner to help those with preventable conditions
- supporting those with long-term conditions or mental health issues
- caring for those with multiple needs as populations age
- getting the best from collective resources so people get care as quickly as possible.
The Integrated Care System in Leicester, Leicestershire and Rutland
We will bring together the NHS, local government, the voluntary and community sectors to work together to tackle the health and care challenges of today and the future, tackling the wider determinants of ill health, reducing inequalities in health and delivering joined – up health and care services, built around the needs of the person.
In LLR we have an excellent track record of working together to improve the health our communities. But we are ambitious: the ICS will build on these foundations to do more to improve the health and wellbeing to ensure everyone in Leicester, Leicestershire and Rutland leads a fulfilling life.
We will focus on:
- Health and wellbeing
Addressing differences in life expectancies, helping people to make more informed lifestyles choices, such as those around smoking and obesity, and ensuring everyone can access appropriate health and care close to their home.
- The best possible care
Ensuring hospitals and community health and care providers are supported to keep up with demand and continue to provide high quality services. We want to do more to reduce waits for some treatments and procedures as well as improve access to primary care and support people to live independently for as long as possible.
- Finance and funding
Health and care costs are increasing – our old buildings are expensive to maintain, it’s more difficult to recruit staff and keep them, and there is greater demand on services than ever before. Working together allows us to share resources and plan services in a more efficient, cost-effective way, that is also better for the communities we serve.
